Billing
Ophthalmology Surgery Center of Illinois has separate financial and billing policies and procedures from your surgeon’s office. You are going to receive separate bills. One from Kovach Eye Institute for the professional surgeon services, one from Ophthalmology Surgery Center of Illinois for your surgical facility fee and one from the anesthesia company.
Be advised your services will be billed to your insurance company. However, you are responsible for the payment of your in-network/out-of-network deductible, co-payment and/or co-insurance at the time of your procedure. These fees are mandated by your insurance carrier and cannot be waived. Please be prepared to pay these fees at the time of service.
Insurance
Ophthalmology Surgery Center of Illinois accepts most major commercial insurance, Medicare and Medicaid. Pre-certification is done by the surgery center prior to scheduling surgery.
